If you need a sick note please contact reception to book a face-to-face appointment with a GP.
If you cannot wait until our next appointment then please call before 12pm, Monday to Friday to arrange a call back from the GP.
If you are off for less than 7 days, you do not need a GP sick note. Please use this link for a Self Certificate.
